Record and assess the hazardBuild the case on facts first.Use specific, factual language. Record what is present, where it is, who or what could be affected, and any immediate circumstances that matter.
A useful record should make sense to someone who was not standing beside you. Add enough location detail to identify the area without relying on memory.
Use your organisation's approved method where one exists. Record the reasoning behind the assessment, not just the final number or label.
If action is taken straight away, document it. That preserves the sequence between discovery, control and the longer-term corrective response.

Build evidence that explains the issueUseful evidence needs context.Start with an image that establishes where the issue sits in the environment, then add closer views that show the detail.
A photograph should not have to speak for itself. Record what the image shows and why that detail matters to the case.
Keep evidence focused on the workplace issue. Do not deliberately capture personal or sensitive information that is not needed for the safety record.

Follow corrective action throughKeep the decision trail intact.An immediate control may reduce exposure without resolving the underlying issue. Record both so temporary measures are not mistaken for closure.
Describe what should be different when the corrective work is complete. This makes later review much clearer.
Keep notes, evidence and decisions in sequence so the record shows what happened between identification and completion.
Where your process requires it, reassess the risk after controls or corrective work and record why the position has changed.

Produce a report people can useMake the record easy to review.Confirm that the description, assessment, evidence, actions and chronology tell the same story and that obvious gaps have been resolved.
A PDF is suited to review, communication and retaining a human-readable record of the case.
CSV export is useful when case data needs to be handled separately in a spreadsheet or another approved business process.

Protect, back up and recover recordsKeep control of the information you create.Protect access with Face ID, Touch ID or the device passcode where supported, particularly when the device is used in shared or mobile environments.
Core case records are designed around the device rather than a compulsory hosted workspace. Export and backup are deliberate user-controlled actions.
If your workflow relies on records remaining available after device loss or replacement, use the backup options appropriate to your HazardHub setup.
